What to Expect at your Appointment

Please Bring the Following to Your Appointment

When you arrive at a Westside health center, please have the following ready:

  • A Photo ID (any picture identification card, such as a driver’s license or work ID)
  • Your medications & dietary supplements that you are taking now.
  • Information about past or present health problems, surgeries, family health history and immunization records.
  • Health insurance card (if you have one).
  • Co-payment.

If you don’t have health insurance, please bring proof of income. It will be used to help you enroll in an insurance program or sliding fee scale if necessary.  Examples of proof of income include 4 consecutive pay stubs, W2, bank statement, most recently filed tax return, Social Security Determination Letter or letter from your employer. Your First Visit

New patients can save time during their first appointment by completing the Patient Registration form prior to your visit.  If you have access to a printer, please print, complete and bring these forms to your appointment.

Telehealth Appointments

Telehealth visits take place in the comfort of your home, or wherever you are – no need to wait in waiting rooms or travel to your appointment.  You can have a telehealth visit using your smartphone, tablet, or computer with a camera and microphone.
