Nutrition Counseling

As a patient of Westside, you have access to nutrition counseling. Our Registered Dietitians work closely with you to access healthy foods and learn the skills needed to live and eat healthily.  Nutrition counseling is available to patients of all ages.

Certified Diabetes Education

Our Certified Diabetes Educator provides patients living with diabetes the resources you need to control and manage your diabetes. We offer diabetes education classes and encourage you to set self-management goals that fit your lifestyle.

Care Coordination

Our healthcare coordinators work with you hand in hand to help connect you to the additional specialty care services as needed. These services help treat and manage your acute conditions and chronic disease.

Social Support Services

As a patient of Westside, you have access to a team of social service coordinators. The coordinators will help you access resources for food, housing, employment, and additional social support programs.

Cancer Screening Navigators

Westside’s Cancer Screening Navigation Program helps patients navigate the cancer screening process for early cancer screening, detection, and diagnosis of breast, ovarian, or colorectal cancers.

Reach Out and Read Program

Our Reach Out and Read program connects health and early childhood literacy. Our providers use children’s books as a tool to measure developmental milestones and encourage parent engagement. At their routine visit, each child gets to take a book to build their own library at home.

Reach Out and Read is a national organization that supports these type of local programs.

Language Translation and Interpretation Services

Individuals can access care in the language that you are most comfortable with.  We have staff members that speak many different languages.  We also use a telephone-based language interpretation service and provide video access to American Sign Language interpreters.

Home Visiting Community Health Worker Program

Westside’s Home Visiting Community Health Worker Program works to improve maternal and child health before, during, and after pregnancy, and to promote child growth and development.
