School-Based Health Center
Westside Family Healthcare at Eastside Charter School
A School-Based Health Center (SBHC) provides convenient health services for students enrolled at the school. The health center is located on campus and staffed by Westside Family Healthcare providers.
Students at EastSide Charter School can walk down the hall to receive care during the school day, making it easier to address health concerns early so they can stay healthy, focused, and ready to learn.
The SBHC does not replace your child’s regular doctor. Instead, our team works with and supports your child’s primary care provider to help coordinate care and ensure students receive the support they need.
Services Provided:
- Sport Physicals
- Health screenings
- Immunizations
- Diagnosis and treatment of minor illnesses and injuries
- Behavioral Health
- Crisis intervention
- Health education/risk reduction
What Parents Should Know:
- Written permission from a parent or guardian is required before services can be provided.
- Student confidentiality is protected. School and medical records are kept separate.
- Services are available regardless of insurance status or a family’s ability to pay.
How To Enroll:
To enroll your child in the School-Based Health Center, please complete and return the following forms:
- Consent Form
- Student Registration Form and Health History Form
- Up-to-date insurance information (if your child is insured)
The completed enrollment forms should be returned to the SBHC.
