What’s a Medical Home?
Being a patient at Westside, means you’re part of a Primary Care Medical Home.
A Primary Care Medical Home is not a special building. It is a way to provide healthcare that puts the patient at the center of health care decision-making.
You choose your own primary care clinician, who will work with the other members of your health care team (including you) to coordinate and personalize your care. This unique partnership means that you will always have active involvement in your own health care plans.
As a medical home, this means that your care should be:
Accessible
Weekend, evening, and same-day appointments are available; a care team member is available by phone 24 hours a day.
Patient Centered
Patients and families have a say in decisions about their health care.
Continuous
You stay with the same healthcare team from year to year. However, if you choose to see one of our other clinicians, your information is in our electronic medical record so your clinician will be able to see it right away.
Comprehensive
Our care team members provide information about your health, services to help you to stay healthy, and care when you are sick. They can also help you find programs in your community that can help you take care of yourself.
Coordinated
Our care team members work to make sure that all of the providers involved in your care have the information about you that they need to be able to address your health care needs.
