The Dental Hygienist is a licensed primary oral health care professional whose goal is to assist Westside Family Health patients in achieving and maintaining optimum oral health. The Dental Hygienist cleans teeth and examines mouth, head, and neck for signs of oral disease. May educate patients on oral hygiene, take and develop x-rays, or apply fluoride or sealants.

CORE RESPONSIBILITIES

  1. Examine and thoroughly document oral conditions, including those of the head and neck region, through the use of clerical measurement, radiographs and other assessment methods
  2. Perform thorough removal of hard and soft deposit with ultrasonic/hand instrumentation and polishing of supra gingival crowns
  3. Review and record patient medical histories
  4. Develop individualized oral hygiene programs for home care
  5. Educate patients, the general public and special population groups particularly the children regarding oral health
  6. Maintain inventory of dental hygiene supplies
  7. Assist other dental staff when necessary
Requirements

MINIMUM QUALIFICATIONS

  1. High school diploma or GED
  2. Graduate of an accredited Dental Hygiene Program
  3. Current Delaware dental hygiene license – Dental hygienists can apply for license by reciprocity if they hold a current Dental Hygienist license in another jurisdiction and have practiced as a Hygienist at least three of the past five years.
  4. Current with continuing education
  5. Basic life support certification

The Dental Assistants are responsible for providing hands-on support to Dentists and completing prep work so they can more efficiently care for patients. Their duties include completing intake paperwork and updating patient records, sterilizing, and organizing dental tools and assisting during dental procedures.

CORE RESPONSIBILITIES

  1. Escort patients to and from rooms.
  2. Ensure proper treatment notes are recorded in the patient’s chart.
  3. Perform clinical procedures as practice act allows and as directed by dentist.
  4. Give patient instruction and demonstrate when necessary.
  5. Monitor patient flow.
  6. Keep dental units ready, stocked, and clean.
  7. Sterilize all instruments and handpieces according to the company’s protocol.
  8. Keep trays, instruments and treatment room drawers organized.
  9. Arrange patient charts and radiographs for the next day’s appointments.

RESPONSIBILITIES:

  1. Room patients, this includes but not limited to: escort patients to examination rooms, prepare them for the provider by completing medication reconciliation, obtaining and recording in the electronic medical record system the patient’s medical history, chief complaint, assess vitals, complete care gaps and any additional care as needed or requested.
  2. Support telehealth visits: similar to rooming patients, the MA will prepare the patient for the provider portion of the visit, this includes identifying the chief complaint, completing a medication reconciliation, addressing care gaps, confirming the patient’s pharmacy
  3. Prepare exam rooms for patient exams, ensuring that exam rooms are neat, clean, well-stocked and properly prepared.
  4. Collect urine, blood, tissue, or other laboratory specimens, log the specimens, and prepare them for testing
  5. Operate electrocardiogram (EKG) or other equipment to administer routine diagnostic tests
  6. Prepare and administer medications and vaccines as directed by the provider with appropriate written orders
  7. Provide education to patients regarding treatment procedures, medications, diets, providers’ instructions and preparation necessary for diagnostic tests, x-rays and laboratory examinations
  8. Clean and sterilize instruments and dispose of contaminated supplies
  9. Perform and document point-of-care equipment testing daily before beginning patient care
Requirements

MINIMUM OBJECTIVE QUALIFICATIONS

  1. High school diploma or GED
  2. Successful completion of a medical assistant training program at an accredited institution
  3. Basic life support certification
  4. Experience using Microsoft Office software and electronic medical record software
  5. Availability to work evenings until 9 p.m. and occasional Saturday morning
  6. Reliable transportation and ability to travel independently between sites

PREFERRED OBJECTIVE QUALIFICATIONS

  1. Some advanced training/college courses in Allied Health, Science, Medical Assisting, or other healthcare-related field
  2. Medical Assistant certification by a national organization accredited by the National Commission for Certifying Agencies (NCCA) or the American National Standards Institute (ANSI) completed within 12 months of hire
  3. Completion of an interpreter training course, certification by the Certification Commission for Healthcare Interpreters or certification by the National Board of Certification for Medical Interpreters

RESPONSIBILITIES OF THE HOME VISITING COMMUNITY HEALTH WORKER:

  1. Providing education and resources with the goal of reducing maternal health disparities and improving infant health in collaboration with home visiting programs in Delaware.
  2. Plan and execute activities that strengthen partnerships and community engagement.
  3. Conduct outreach activities in targeted zones, focusing on reaching high-risk women and children up to 5 years.
  4. Establish trusting relationships with patients and their families while providing general support and encouragement.
  5. Collaborate with other healthcare professionals and community organizations to coordinate comprehensive care and support services.
  6. Traveling between our Dover and Wilmington sites and communities throughout Delaware to conduct meetings, and outreach events in the community with individuals and established patients to provide case management.
  7. Complete screenings and needs assessment, identifying social needs, developing effective service plans.
  8. Maintain accurate and timely documentation.
Requirements

MINIMUM OBJECTIVE QUALIFICATIONS

  1. Associate’s degree or equivalent experience.
  2. Fluency in written and spoken English and Spanish and/or Haitian Creole.
  3. One year of related work or volunteer experience
  4. Knowledge of community resources appropriate to the needs of patients/families
  5. Six months of experience using computers and Microsoft Office software
  6. Driver’s license and clean driving record
  7. Ability to work weekends, evenings and nights as needed
  8. Ability to travel independently between offices and communities throughout Delaware

The Behavioral Health Counselor at Westside provides counseling with an emphasis on prevention, working with individuals and groups to promote optimum behavioral, mental and emotional health. The Behavioral Health Counselor may help individuals deal with issues associated with addictions and substance abuse; family, parenting, and marital problems; stress management; self-esteem; and aging.

CORE RESPONSIBILITIES

  1. Collect information about clients through interviews, observation, review of medical records and/or tests and prepare and maintain all required treatment records and reports, entering information into the electronic medical records system in accordance with policy.
  2. Evaluate clients’ physical or mental condition and develop, implement and modify treatment plans.
  3. Counsel clients or patients, individually or in group sessions, to assist in overcoming dependencies, adjusting to life, or developing skills or strategies for dealing with their problems.
  4. Act as client advocate to coordinate required services or to resolve emergency problems in crisis situations.
  5. Collaborate with other staff members to perform clinical assessments or develop treatment plans.
  6. Gather information about community mental health needs or resources that could be used in conjunction with therapy and refer patients, clients, or family members to community resources or to specialists as necessary.
  7. Assist the primary care and behavioral health teams in developing care management processes such as the use of screenings, disease management techniques, case management and patient education.
Requirements
  • Master’s degree in mental health counseling, social work, psychology or a related healthcare, social science or human services field
  • Delaware licensure in a counseling field (i.e., Licensed Clinical Social Worker, Licensed Professional Counselor of Mental Health, Licensed Associate Counselor of Mental Health)
  • One year of social work and behavioral health experience
