Westside Family Healthcare’s chronic disease care coordinators to assist diabetic patients with scheduling on-site medical appointments with a podiatrist and support sessions with a registered dietitian and/or a certified diabetes educator, as well as accessing affordable insulin and tools to manage their diabetes.
Diabetes is a chronic disease that influences an individual’s blood sugar levels that has an impact on an individual’s ability to stay healthy. When an individual’s blood sugar levels are too high and abnormal, individual’s face health complications such as, heart disease, stroke, and blindness.
